COORDINATION CONNECT is a highly respected, established and independent NDIS registered provider of Support Coordination Level 2 and Specialist Support Coordination services.

How can COORDINATION CONNECT support Participants?

  • Work with you to find the best communication method to support you engage with us and others,

  • Assist in exploring options to connect with services and supports,

  • Assist you action your NDIS Participant Plan goals,

  • Review the progress of achieving goals,

  • Support you to prepare for NDIS plan Reviews and/or Changes in Circumstances,

  • Assist you to exercise your choice and control,

  • Assist you explore and connect with community and/or alternative support options,

  • Strengthen your support networks,

  • Support you to learn about and/or utilise your NDIS Participant Portal,

  • Support you to set up and understand service agreements with services,

  • Support you through any difficulties experienced, including points of crisis.

  • Understand what is and isn't funded by the NDIS.
