Compliments, Complaints and Suggestions:

The National Disability Insurance Scheme (Complaints Management and Resolution) Rules 2018 require registered NDIS providers to have an effective system for management and resolution of complaints about the supports or services they provide.

Your feedback, compliments and complaints about all aspects of our service delivery are very important to us.

Should you wish to give a compliment, provide us with feedback or make a complaint about any part of the service we provide to you, please provide us with this information by:

  • Write to us at Coordination Connect, PO Box 214, Sorell, Tasmania, 7172

  • Phone us on 0404839286 and speak with one of our Directors, Adam or Jenny. (A copy of your complaint, compliment or feedback will be recorded and sent to you for confirmation that the information was recording accurately so a response can be planned with you/provided to you once confirmed).

  • Complete this online form https://coordinationconnect.snapforms.com.au/form/coordination-connect-support-coordination-satisfaction-and-feedback-survey

  • Contact an Advocacy service to assist you with completing your compliment, complaint or feedback.(see details below).

  • Contact your Nominee, Guardian, Person Responsible or a friend or carer to assist you with completing your compliment, complaint or feedback.

Your compliment, complaint or feedback will be responded to in 5 working days from the date of receipt by one of the Directors Adam and Jenny. They will work with you to resolve your complaint and/or support you to access additional assistance to assist you resolve the complaint e.g. referral to advocacy services if you would like this to help you.

If during or after contact with Coordination Connect you feel your suggestion, compliment or complaint has not or is not being addressed appropriately:

  • Contact the NDIS Quality and Safeguards Commission on 1800 035 544

  • See a staff member at your local NDIA Office
